The American Cancer Society runs a program, staffed by volunteers, to offer cancer patients in need rides to treatments and appointments.

The form below has Munson Medical Center filled in – just change to the relevant healthcare system  as needed and use.   Directions on where to send the form are at the bottom.

Please note that while the program is offered in most Northern Michigan areas, there is a dependence on volunteer drivers.

MI PATH Program- Chronic Disease Self-Management     

The PATH program (also known as the Stanford Chronic Disease Self-Management Program) was developed to help people learn techniques and strategies for the day-to-day management of chronic or long term health conditions. It is a six-week workshop conducted in 2 1/2 hour sessions each week. Workshops are offered in convenient, easily accessible community locations and are offered for free, or at very low cost to participants. Click here for additional information and local contact information.
